Signpost has an excellent booklet for families in West Lothian.  Called Are you prepared for change? it covers issues on preparing to leave school.  The booklet is available free of charge by contacting them.

The NAS Scotland transitions outreach service provides advice, training and support for young people with autism about to leave school and move into adult life.  More information available from the outreach service manager at 0141 221 8090.

Transforming Transitions offers a free person centred plan putting the views, hopes and aspirations of the child or young person first in planning for the next stage in their lives.

CCNUK promotes and supports key working for families with a disabled child or young person so that their views are listened to and agencies co-ordinate services effectively.
